2010 Lookout Mountain Recreational Football

 

 

            The recreation department will offer a fun football program for children in the 4th, 5th and 6th grades starting Tuesday Sept.7th.  The program will center on the basics of the game and will be full contact. Players are responsible for their own equipment; however, the recreation department does have a good bit of equipment to loan out. For those needing equipment there will be an equipment day Wednesday Sept. 1st (3-4pm) at LMS gym.
            Practice will be on Monday, Tuesday and Wednesday from 3:15 until 4:30 at the Lookout Mountain School football field. Games will be on Mondays and Wednesdays and will start around 3:30. There will be no games or practice on October 18th, 19th, 20th due to Fall Break. After a few weeks of practice the children will be placed on teams and a schedule will be given out. Please drop players off in the skating rink parking lot and pick them up on time in front of the school. The season will run through the first of November. There is a $25 registration fee for each child participating. Please fill out and sign the parental authorization and bring form and check to the first practice or mail to Lookout Mountain Recreation Board, P.O. Box 413, Lookout Mountain, TN 37350. Please make checks payable to the Lookout Mountain Recreation Board. No player will be turned away because of fees. For Information call 821-6212.

 

Parental Authorization

 

 

            I, parent or guardian of the below named candidate for a position in the above mentioned sports program, hereby give approval to the child’s participation in any and all league activities during the current season. I assume all risks and hazards incidental to such participation including transportation to and from activities, and do hereby waive, release, absolve, indemnify and agree to hold harmless the parent or local league organization, the organizers, sponsors, supervisors, participants and persons transporting the child to and from activities, for any claim arising out of an injury to the child, except to the extent and in the amount covered by accident and/or liability insurance held by the local league.

            I also grant permission to managing personnel or other league representatives to authorize and obtain medical care from any licensed physician, hospital or medical clinic should the child become ill or injured while participating in league activities away from home, or at other times when neither parent is available to grant authorization for emergency treatment.
